Salary: £24,214 – £28,000 per annum

The West Berkshire Rural Primary Care Network (PCN) comprising Kintbury and Woolton Hill, Hungerford and Lambourn Surgeries, is looking for a Social Prescribing Link Worker to join their teams of GPs, Practice Nurses and Health Care Assistants.

This is an exciting new role working for a small, rural and friendly PCN where the post-holder will join multi-disciplinary teams at our GP surgeries and create an effective and collaborative approach to patients becoming more involved in their personal care.

Our ideal candidate is someone who is self-motivated and able to work flexibly during NHS core hours, with some evening and Saturday morning shifts. They will seek to develop the potential of people, families and communities in creating better health access, outcomes and experiences by encouraging our patients to take more control over the way their care is planned and delivered. They will either work with patients in each of the practices or with patients in their own homes. Use of a car and a clean driving license and an enhanced DBS check are, therefore, requirements of the post.

We welcome applications from both experienced Social Prescribing Link Workers and candidates with social service or relevant experience, who are keen to develop their skills in the primary care environment. An understanding of NHS Englands’ components of personalised care and the ability to provide motivational and behavioural techniques would be a distinct advantage. However training will be provided to the right candidate.
