NEDPCN is an organisational group of four GP practices in North East Derbyshire. The Practices are Barlborough Medical Practice, Killamarsh Medical Practice, The Springs Medical Partnership and The Valleys Medical Partnership. The PCN is committed to improving the care of patients in North East Derbyshire and sustaining General Practice.

We are fortunate to be working with Derbyshire Voluntary action to take forward Social Prescribing in this area. Social prescribing empowers people to take control of their health and well being through referral to ‘link workers’ who give time, focus on ‘what matters to me’ and take a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.

If you are a creative, innovative thinker, who would love the challenge of working across exciting new partnerships within the Primary Care Network and working collaboratively with NHS and Voluntary Sector colleagues in North East Derbyshire, this could be the job for you.

The post will be employed by NEDPCN and have access to the NHS Pension scheme, but will be part of the team of Link Workers at Derbyshire Voluntary Action’s ‘Connect to…’ social prescribing service. You will work closely with the GP practice teams and the Clinical and Management leads for the project.

You will adopt an holistic, non-medical, person-centred approach to your caseload, co-designing action plans to improve health and wellbeing based on ‘what matters’ to the person and their individual strengths and needs – and supporting them to achieve their goals.

Ideally you will have worked in a similar role before, but this position would also be suited to someone with good local knowledge, appropriate experience and enthusiasm. If you are proactive, enthusiastic, person-centred and hold a clean driving licence with unlimited access to a car, then we would love to hear from you.
